Referral Program Terms and Conditions

I. Program Eligibility
This Referral Program (the “Program”) is available exclusively to current clients of Preventty – USA Specialty Insurance (the “Company”). Eligible participants (“Referrers”) may refer new clients who have not previously held any insurance policy with the Company. Employees, contractors, and affiliates of the Company, as well as their immediate family members, are not eligible to participate in this Program.

II. Referral Submission Process
To participate in the Program, Referrers must share the Company’s contact information with individuals within their professional network, including but not limited to truck drivers, colleagues, or other relevant contacts (each, a “Referral”). In order for the referral to be considered valid, the Referral must explicitly mention the Referrer’s name during their initial communication with the Company, ensuring proper attribution of the referral. The Company reserves the right to verify the Referrer’s involvement in the referral process prior to issuing any rewards.

III. Rewards
Following the successful activation of an insurance policy by a Referral, the Referrer will be awarded a $30 gift card, redeemable at either Amazon or Walmart, based on their selected preference. There is no restriction on the number of referrals a Referrer may make, and each qualified referral will earn an additional $30 gift card.

IV. Referral Eligibility Criteria
To qualify for the reward, the Referral must purchase and activate a new insurance policy with the Company. The policy must remain active and in good standing for a minimum of thirty (30) consecutive days. Policies canceled within the first 30 days will result in disqualification for the reward. Only after meeting these criteria will the Referrer become eligible to receive the gift card.

V. Issuance of Gift Cards
Gift cards will be issued within fourteen (14) business days following the confirmation that the Referral’s policy has remained active and in good standing for the required 30-day period. The Company will deliver the gift card either via email or standard mail, according to the delivery method selected by the Referrer during the reward redemption process.

VI. Limitations and Restrictions
Gift cards issued under this Program are strictly non-transferable, non-refundable, and cannot be exchanged or redeemed for cash or any alternative form of compensation. Additionally, gift cards may not be used to purchase other products, services, or applied towards any outstanding balances with the Company. Once a gift card has been delivered to the Referrer, the Company assumes no liability for any loss, theft, or misuse of the gift card, including unauthorized use, after delivery. It is the Referrer’s sole responsibility to safeguard the gift card and ensure its proper use.

VII. Program Modifications
The Company reserves the exclusive right to modify, suspend, or terminate this Program at any time without prior notice. Any modifications will not affect referrals made prior to the effective date of the change. In the event of Program termination, the Company will honor all valid referrals made before the termination date.

VIII. Governing Law and Jurisdiction
These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of law principles. Any disputes arising out of or related to this Program shall be subject to the exclusive jurisdiction of the courts located in Miami-Dade County, Florida.

IX. Acceptance of Terms
By participating in the Program, Referrers acknowledge and agree to be bound by these Terms and Conditions. Any violation of these Terms may result in disqualification from the Program and forfeiture of any rewards earned.
